Form Instructions

This form allows you to submit your team members for each course played during the 2026 league season.

1.Enter Your Name in submitted by box, (you are Player 1)

2. Enter your Group Code (A, B, C, D)

3. Enter the other three players in your group for the course(s) checked. If you have less than 3 players, we will assign a fourth player to your group

4. Click "Submit Teams" bottom of page to send data.
