Form Instructions

This form allows you to submit your team members for each course played during the 2027 league season.

1.Enter Your Name in submitted by box, (you are Player 1)

2. Enter your Group Code (A, B, C, D), Willie, Foster group A or B, Jessie group C and Jim group D

Note: Group C & D alternates between Shoaff and McMillen

3. Enter the other three players in your group for the course(s) checked. If you have less than 3 players, we will assign a fourth player to your group

4. Click "Submit Teams" bottom of page to send data.
